Brief Intake Form: Before your consultation, you’ll complete a short, secure intake form to share your symptoms, health history, and wellness goals. This helps us focus your visit on what matters most to you.

Comprehensive Functional Lab Panel: You’ll visit your local Quest Diagnostics for a blood draw prior to your consultation. This panel evaluates key markers that influence energy, mood, metabolism, and hormones—including thyroid function, nutrients, and inflammation.

60-Minute Consultation: Once your lab results are in, we’ll meet virtually to review your data in detail, connect the dots between your symptoms and underlying imbalances, and discuss your personalized recommendations.

Customized Wellness Plan: You’ll receive a clear, actionable plan that outlines your next steps for improving energy, balance, and overall well-being—whether that’s optimizing nutrition, exploring hormone support, or continuing care through our functional medicine programs.

Your Clarity Consult includes a comprehensive functional lab panel and a 60-minute consultation to review your results in detail with Jen Piermarini, FNP-C.
You’ll receive a lab requisition to complete your blood draw at a nearby Quest Diagnostics location. Quest will bill your insurance directly for the lab work, and any costs or copays will be determined by your individual insurance plan.
Once your results are available, we’ll meet virtually to review your data, connect the dots between your symptoms and lab findings, and outline a clear, personalized plan for improving your health and restoring balance.
